Job summary

Creative Solutions In HealthCare is seeking a Full-Time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will provide person-centered care to help residents improve mobility, strength, and quality of life.

The PTA will administer prescribed treatments under the supervision of a Physical Therapist, document progress, and collaborate with the team to adjust plans for optimal outcomes. Texas license required; SNF/long-term care experience preferred.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• In good standing with regulatory boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health EHR.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services and interventions under supervision of a Physical Therapist.
• Respond to requests for service and relay information to the Physical Therapist.
• Implement and modify treatment plans under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Ensure thorough and timely documentation in line with policies and regulations.
• Attend required Rehab Director meetings.
