Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to join our in-house skilled nursing facility partner in Torrington, CT. You will support the Physical Therapist in delivering patient-centered therapy services and contribute to resident well-being and mobility goals.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in developing target goals and treatment plans.
• Assist in selecting therapy techniques/media and adapt as indicated by resident performance.
• Monitor residents’ response to intervention and consults with the Physical Therapist as needed.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and team.
