Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant to join our in-house Skilled Nursing Facility partner in Bradford, PA. You will work under the supervision of the Physical Therapist to provide therapy services, conduct screenings, assist in evaluations, and help implement individualized treatment plans.

Formación

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluation and development of target goals and treatment plans with the Physical Therapist.
• Implement individual and group treatment plans including ROM, gait, balance, strengthening and modalities.
• Document compliant and medically necessary therapy in NetHealth EMR per CMS expectations.
• Educate residents, families and staff on therapy plans and safety.
• Coordinate schedules with the PT and care team to maximize productivity.
