PureHealth is seeking a licensed Physical Therapy Assistant to provide physical therapy services under supervision in a post-acute setting. The role supports the Physical Therapist, participates in assessments, and contributes to patient-centered treatment plans.

Responsibilities include documentation, education, and collaborative discharge planning, ensuring adherence to policies and state regulations. Regular shifts in a fast-paced team environment are expected.

Qualifications

• Graduate of an accredited therapy assistant program.
• Minimum of one year therapy assistant experience.
• Excellent communication and collaboration within an interdisciplinary team.

Responsibilities

• Under the Physical Therapist, assist assessment processes for customers.
• Provide interventions aligned with the treatment plan and customer needs.
• Accurately document daily charges, progress notes, and reports.
• Monitor and adjust treatment based on patient response and progress.
• Educate customers and families; document learning needs and outcomes.
• Collaborate with team for discharge planning and care coordination.
