PureHealth, a post-acute care provider in Texas, seeks a Licensed Physical Therapy Assistant to deliver services under a Physical Therapist's supervision, following PureHealth policies and state and federal regulations.

Responsibilities include assessment contributions, treatment planning support, accurate documentation, patient education, and coordinating care with the broader team to ensure a comprehensive view of each customer’s health.

Qualifications

• Graduate of an accredited therapy assistant program.
• Minimum one year PTA experience preferred.

Responsibilities

• Provide PT services under supervision per state regulations.
• Document daily charges and progress notes accurately.
• Monitor treatment progress and adjust plans as needed.
• Educate customers and families about care plans.
• Collaborate with team for discharge planning and updates.
• Demonstrate unit-specific clinical competencies annually.
• Schedule management to meet department productivity targets.
• Contribute to program development and quality improvement.
• Ensure compliance with HIPAA and corporate policies.
• Maintain effective communication with internal and external stakeholders
