Job summary

Home Health Care, Inc. seeks a Physical Therapist Assistant (PTA) to deliver one-on-one therapy in patients’ homes across Saint Cloud, Sauk Centre, and Central Minnesota. You will travel to patients and implement plans of care under a supervising Physical Therapist.

Qualifications

• Active Minnesota Physical Therapist Assistant (PTA) license.
• Strong therapeutic exercise and functional mobility skills.
• Excellent patient and caregiver communication.
• Ability to travel throughout the St. Cloud & Sauk Centre territory.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide one-on-one Physical Therapy treatments in patients' homes.
• Carry out individualized plans of care.
• Provide therapeutic exercise and strengthening.
• Perform gait and mobility training.
• Educate patients and caregivers on safe mobility and techniques.
• Monitor patient response and communicate changes in condition.
• Collaborate with nurses, OTs, SLPs, physicians, and other clinicians.
• Complete timely and accurate clinical documentation.
