Job summary

Home Health Care, Inc. is seeking a Physical Therapist Assistant (PTA) to deliver one-on-one therapeutic services in patients’ homes throughout the Western Twin Cities Metro and nearby communities. You will operate under the supervising PT and implement plans of care in a field-based setting.

The role emphasizes mobility, balance, and functional independence, with productivity tied to per-visit earnings and mileage reimbursement. Excellent collaboration with the care team is expected.

Qualifications

• Active Minnesota PTA license required.
• Experience in home health or field-based therapy preferred.
• Strong ability to educate patients and caregivers.

Responsibilities

• Provide one-on-one Physical Therapy in patients' homes.
• Carry out individualized plans of care established by the PT.
• Provide therapeutic exercise and strengthening.
• Perform gait, balance, transfer and mobility training.
• Help improve endurance, coordination and functional independence.
• Educate patients and caregivers on safe mobility and techniques.
• Monitor patient response to treatment and communicate changes.
• Provide progress updates to supervising PT.
• Collaborate with nursing, OT, SLP, physicians and clinicians.
• Complete timely and accurate clinical documentation.
