Job summary

Horizon Health Network is seeking Home Health Aides (HHAs) to provide in-home care across Piqua, OH and surrounding areas. You will assist clients with daily living activities, monitor health changes, and offer companionship in a respectful, patient-centered environment.

The role supports flexible scheduling (Full-Time, Part-Time, PRN) and ongoing training. A valid Ohio STNA or CNA license and a driver’s license are required to start.

Qualifications

• Current active Ohio STNA license or Ohio CNA license (within 2 years).
• One year of verifiable caregiving experience under RN supervision.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assist clients with activities of daily living (bathing, dressing, grooming, toileting).
• Support safe transfers, ambulation, and mobility.
• Prepare light meals and assist with feeding as needed.
• Perform light housekeeping and laundry related to client care.
• Monitor changes in client condition and report to supervising nurse.

Responsibilities

• Assists clients with activities of daily living, including bathing, dressing, grooming and tolieting
• Helps with safe transfers, ambulation and mobility
• Prepares light meals and assists with feeding as necessary
• Performs light housekeeping, laundry, and other household tasks related to the client's care
• Monitor and report changes in the client's condition to the supervising nurse and care team
• Provide companionship and emotional support
• Maintain accurate documentation of care provided

Certifications/Requirements (must have one of the following)

• Current active Ohio State Tested Nursing Assistant (STNA) license
• Ohio Certified Nursing Assistant (CNA) license - less then two years old
• One year of verifiable experience with the last two years that is underneath the supervision of an RN.

Other Qualifications

• Valid driver's license, reliable transportation, and proof of auto insurance
• Ability to work independently while following a personalized care plan
• Excellent communication and interpersonal skills
• Previous caregiving or home health experience is preferred but not required
• First aid certification is preferred but not required
