Job summary

Alternate Solutions Health Network is seeking a skilled Physical Therapist Assistant to provide in-home PTA services under a Physical Therapist plan of care. You will document progress, perform prescribed therapies, and communicate clearly with patients, families, and the care team.

The role requires an active PTA license in Ohio, at least one year of experience (preferably in acute or home care), and a valid driver’s license. Mileage is reimbursed and a comprehensive benefits package is offered.

Qualifications

• Licensed Physical Therapist Assistant (PTA) in the state of employment is required.
• Minimum of one year of PTA experience, preferably in acute or home care settings.
• Strong communication with patients, families, physicians and team members.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled PTA services at patients' homes per the plan of care.
• Document patient progress, observations, and changes promptly.
• Perform PT procedures to relieve pain and restore function.
• Educate patients and families on therapy techniques and safety.
• Coordinate with physicians, therapists and schedulers; participate in case conferences.
