Job summary

Supplied Talent seeks a licensed Physical Therapist Assistant (PTA) to join a skilled nursing and long-term care rehabilitation team in Colorado. The PTA will deliver individualized therapy to residents under supervision, supporting functional recovery and quality of life.

Responsibilities include implementing treatment plans, gait/balance training, progress documentation, and family education on home programs. A strong patient-first mindset and interoperability with the care team are essential.

Qualifications

• Associate degree from an accredited PTA program.
• Current, unrestricted PTA licensure in the state (Colorado) or ability to obtain before start date.
• Current CPR/BLS certification.
• Strong interpersonal and patient-first communication.

Responsibilities

• Implement PT guidelines and treatment plans under the supervising PT.
• Deliver interventions including therapeutic exercise, gait and balance training, and modality use.
• Monitor and document progress, status, and response to treatment per standards.
• Communicate with the supervising PT and care team about goals and progress.
• Educate patients and families on home exercises, fall prevention, and safe mobility.
• Participate in care planning, utilization reviews, and MDS documentation as applicable.
• Maintain timely clinical documentation in compliance with Medicare/Medicaid and facility rules.
• Uphold resident rights, dignity, and confidentiality at all times.
• Keep the therapy gym clean, organized, and safe.
