Centennial Healthcare seeks a compassionate Physical Therapist Assistant (PTA) to join our Southern Colorado rehabilitation team. The PTA will provide therapy under a licensed PT in SNF/long-term care, document progress, and collaborate with a broad care team to foster resident mobility and independence.

Qualifications

• Current PTA license in Colorado or eligibility through compact licensure.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong communication and documentation skills.

• Ability to work effectively with an interdisciplinary healthcare team.

Responsabilités

• Provide physical therapy treatments under the direction of a licensed Physical Therapist in a skilled nursing and long-term care setting.
• Complete accurate and timely documentation of resident care and progress.
• Assist residents in improving mobility, strength, function, and overall quality of life.
• Collaborate with therapists, nursing staff, physicians, and other healthcare professionals to develop and implement individualized treatment plans.
• Follow all state and federal regulations governing therapy services.
• Adhere to facility policies, procedures, and professional standards.
• Promote resident independence, safety, and successful rehabilitation outcomes.
