• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Assist with patient transfers and ambulation; manage equipment and manual techniques.
• Administer procedures as part of rehab plan, including exercises, modalities, and wound care.
• Provide counseling to patients, families, and other providers about physical disorders.
• Generate discharge summaries compliant with regulations.
