Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver skilled rehabilitation to patients per regulatory standards.
• Assist with standing, walking, lifting, transferring, and repositioning of patients and equipment.
• Administer procedures as part of rehabilitation: manual techniques, exercises, modalities, devices, and wound care.
• Provide counseling to patients, families, and caregivers about physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.
