Emblem Home Health & Hospice in Chandler, AZ is seeking a licensed Physical Therapy Assistant (PTA) to join a full-time team delivering hands-on therapy under the supervision of a Physical Therapist. Qualifications

• Licensed Physical Therapy Assistant in Arizona.
• Graduate of a CAPTE-approved PTA program (passed national exam).
• Current CPR certification required.

Responsibilities

• Deliver physical therapy treatments per PT and physician’s plan of care.
• Guide patients through therapeutic exercises and mobility training.
• Monitor and report patient progress and condition changes.
• Educate patients on mobility aids and prosthetic devices.
• Maintain accurate clinical notes and participate in team meetings.
