Continuum Therapy Partners, LLC is seeking Physical Therapists to join our growing team at Avalon Health and Rehab locations. We value a therapist-led approach and a leadership team that supports your clinical perspective.

Formación

• DPT degree required.
• Current, unencumbered state PT license; NPTE must be passed.
• 1 year of PT experience preferred.

Responsabilidades

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
