Job summary

Sternrehab is seeking a PRN Physical Therapist for Autumn Lake at Ocean View, NJ to guide nursing residents through rehabilitation addressing movement challenges and daily activities.

You'll evaluate, set measurable goals, provide exercises and gait training, select devices, monitor outcomes, and document care while coordinating with nursing and therapists.

This PRN position offers flexible hours with an unlimited referral bonus and collaborative support from therapy leadership.

Qualifications

• Active New Jersey Physical Therapist license.
• Strong assessment skills and practical treatment planning.
• Effective communication and accurate clinical documentation.
• Skilled nursing or post-acute experience is helpful; new graduates are welcome to apply.

Autumn Lake at Ocean View | Ocean View, NJ

$55–$60/hour | PRN

Stern Therapy Consultants is seeking a PRN Physical Therapist at Autumn Lake at Ocean View. Guide skilled nursing residents through rehabilitation that addresses their current movement challenges and prepares them for the activities they need to perform each day.

What You'll Do

• Evaluate strength, balance, walking, transfers, and other aspects of functional mobility.
• Develop measurable treatment goals based on residents' medical needs, prior function, and recovery priorities.
• Provide therapeutic exercise, gait training, and functional activities tailored to individual abilities.
• Determine appropriate assistive devices and teach safe mobility techniques to residents and caregivers.
• Monitor clinical outcomes, adjust plans of care, and supervise PTA services.
• Document evaluations and treatment while collaborating with nursing and fellow therapists on care transitions and discharge planning.
