Autumn Lake at Southgate in Carneys Point, NJ is seeking a PRN Physical Therapist to assess movement, strength, balance, and functional mobility for skilled nursing residents and help them regain daily activity independence.

The position pays $60–$65 per hour with PRN coverage aligned to your availability. You’ll collaborate with therapy leadership, document care accurately, and support discharge planning; new graduates with NJ license are welcome.

Qualifications

• Active New Jersey Physical Therapist license.
• Strong evaluation, clinical reasoning, and treatment planning skills.
• Clear communication and accurate documentation.
• New graduates welcome; skilled nursing/post-acute rehab experience a plus.

Responsabilités

• Assess movement, strength, balance, and functional mobility to identify rehab needs.
• Establish individualized plans of care considering medical precautions and resident goals.
• Provide therapeutic exercise, gait training, and transfer interventions per recovery stage.
• Select mobility devices and educate on safe use.
• Measure progress, revise treatment approaches, and oversee PTA services.
• Complete timely clinical records and support discharge planning.

Stern Therapy Consultants is hiring a PRN Physical Therapist at Autumn Lake at Southgate in Carneys Point, NJ. Use your assessment and treatment skills to help skilled nursing residents recover mobility, navigate physical limitations, and prepare for daily activities with greater confidence.

What You’ll Do

• Assess movement, strength, balance, and functional mobility to identify each resident's rehabilitation needs.
• Establish individualized plans of care that account for medical precautions, prior abilities, and resident goals.
• Provide therapeutic exercise, gait training, and transfer interventions suited to each stage of recovery.
• Select appropriate mobility devices and educate residents and caregivers on safe use.
• Measure progress, revise treatment approaches, and oversee PTA services within the plan of care.
• Complete timely clinical records and work with nursing and rehabilitation colleagues on mobility recommendations and discharge planning.
