Continuum Therapy Partners is seeking a Physical Therapist to provide PRN coverage at Trinity Care Center in Farmington, MN. The role emphasizes continuity and adherence to the same clinical standards as the full-time team, with flexible scheduling to fit your availability.

Qualified candidates hold a Doctor of Physical Therapy, an active state license, and NPTE eligibility. Prior PT experience is preferred, and you will collaborate with other disciplines to optimize patient outcomes while

• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
