Job summary

Continuum Therapy Partners, LLC is seeking a part-time Physical Therapist for St. Joseph's Villa Nursing and Rehab Home Health Department in Omaha, NE. You will evaluate, develop, and implement individualized treatment plans and coordinate with other disciplines to support patient outcomes.

Requirements: Doctor of Physical Therapy (DPT), current state PT license (must pass NPTE). 1 year PT experience preferred. Benefits include 401k with company match and generous PTO; daytime schedule.

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred.

• Documents patient progress and adjusts plans of care based on clinical outcomes
