Job summary

Continuum Therapy Partners is seeking a part-time Physical Therapist for St. Joseph's Villa Nursing and Rehab Home Health Department in Omaha, NE. A Doctor of Physical Therapy (DPT) is required, with an active NE license and NPTE required for this role.

One year of PT experience is preferred. Responsibilities include evaluating, developing, and implementing individualized plans of care, documenting patient progress, and educating patients and families on home exercise programs and injury

Qualifications

• DPT degree required and eligibility to obtain NE license.
• Current, unencumbered NE PT license; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
• Provide part-time daytime coverage at St. Joseph's Villa Nursing and Rehab Home Health Department.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred
