Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Physical Therapist to join our Cartersville, GA team. The role focuses on 1:1 patient care, evaluating and treating patients in home settings with physician-directed orders and goal-oriented plans.

Must hold a PT license and CPR certification, with opportunity for professional growth. Responsibilities include developing treatment plans to maximize independence, coordinating with families and care teams, and

Formación

• Bachelor's degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets.
• At least 2 years of PT experience; hospice/community experience preferred.

Responsabilidades

• Perform PT evaluation and treatment based on physician orders and procure adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence and function.
• Maintain communication among the treatment team, physician, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care as applicable.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
