Adoration Health, part of the BrightSpring Health Services family, is seeking a Home Health Physical Therapist for the Cartersville, GA area. The role emphasizes 1:1 patient care in a home health setting with a PRN, weekly-visit schedule.

Ideal candidates hold a Bachelor's in Physical Therapy, a current state license, and CPR certification, with 2+ years of PT experience preferred (hospice/community experience a plus).

Qualifications

• Bachelor's Degree in Physical Therapy required.
• New graduates may be considered in select markets.
• At least 2 years of physical therapy experience; hospice/community experience preferred.
• Active license in state of practice; CPR certification required.

Responsibilities

• Perform PT evaluation and treatment based on physician orders.
• Develop measurable goals and treatment plans to maximize independence.
• Communicate with treatment team, patient, and family members.
• Maintain thorough documentation for billing and continuity of care.
• Supervise PT assistants and aides as required by state practice act.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
