Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Physical Therapist to provide patient-centered care in Greeneville, TN. The role offers 1:1 patient focus, flexible scheduling and independence with team support.

Responsibilities include evaluations, goal setting, documentation for billing, and supervision of aides within state guidelines. PRN schedule typical; licensure and CPR required.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• At least 2 years of PT experience; hospice/community preferred.
• Valid license for the state of practice and CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and treatment plans to maximize independence.
• Maintain communication with the treatment team, physician, patient and family.
• Ensure thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides per state practice act.

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
