Adoration Health in Cartersville, GA is seeking a part-time Physical Therapist to join our home health team. You’ll provide patient-centered therapy 20 hours per week in the Cartersville area, delivering 1:1 care and collaborating with a supportive team to promote independence and quality of life.

Qualified candidates will hold a Bachelor’s Degree in Physical Therapy, be licensed in Georgia, and have CPR certification. Experience in home health or hospice is preferred.

• Bachelor’s Degree in Physical Therapy required.
• New graduates may be considered in select markets.
• Georgia license required; CPR certification.
• Home health experience preferred.

Responsabilidades

• Perform physical therapy evaluation and treatment based on physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans toward independence.
• Maintain communication with treatment team, physician, patient, and family.
• Maintain thorough documentation for proper billing.
• Supervise Physical Therapist Assistants and PT Aides as applicable.

Responsibilities

• P erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
