Job summary

Adoration Health in Waycross, GA is seeking a dedicated Home Health Physical Therapist. You will provide one-on-one patient care, evaluate and treat patients, and collaborate with a multidisciplinary team to support independence and quality of life.

The role offers a Monday–Friday schedule with a stable home health setting. Applicants should have a PT license in GA, CPR certification, and 2+ years of relevant experience; new grads considered in select markets.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Valid license in the state of practice and current CPR certification.

Responsibilities

• Perform PT evaluation and treatment based on physician orders and adapt equipment as needed.
• Set measurable goals and develop treatment plans to maximize independence and function.
• Maintain communication among the treatment team, physician, patient, and family.
• Document services thoroughly and timely for proper billing.
• Supervise PTAs and PT aides per state practice act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
