Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking a Full-Time Physical Therapist (PT) to perform home health visits, assessments, and admissions. This role services adult and geriatric clients across territories in Stokes and Surry counties.

As a home care Physical Therapist, you will be an essential part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to stay at home safely and comfortably.

Responsibilities:

• Conduct home visits within designated territories.
• Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents to prevent and promote wound healing.
• Develop and update individualized care plans, including manual therapy, gait training, and balance interventions.
• Assess and revise care plans regularly; collaborate with nursing staff within multidisciplinary teams.
• Educate clients and families on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

• Current North Carolina Physical Therapist (PT) license.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records preferred.
