Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits for our office located in Baltimore, MD. This office services adult and geriatric clients on a per-visit basis in territories throughout Perry Hall.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients within designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Continuously assess and update the care plan, collaborating with nursing staff as appropriate.
5. Educate and instruct clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

1. A current MD Physical Therapist - PT license.
2. Graduation from an American Physical Therapy Association-approved program, as evidenced by transcript or diploma.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Solid computer skills; prior experience with electronic medical records (EMR) is preferred.
