Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our therapist-led network. Based in Streetsboro, Ohio, this PRN daytime position supports our national footprint and emphasizes individualized patient care in collaboration with a leadership team that values your professional perspective.

Candidates must hold a Doctor of Physical Therapy, a current Ohio license, and NPTE eligibility; one year of experience is preferred.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered Ohio license; NPTE eligibility required.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
