Job summary

Lincoln Park Care Center in New Jersey is seeking a PT Physical Therapist to join our sub-acute rehabilitation team. The role focuses on delivering care per personalized care plans, collaborating with an interdisciplinary team, and helping residents achieve functional goals in a supportive environment.

Requirements include a NJ PT license, at least one year in sub-acute/long-term care, familiarity with Medicare/Medicaid reimbursement, and weekend availability.

Qualifications

• A valid New Jersey Physical Therapy PT license and a minimum of one year of experience in a sub-acute and long-term care setting is required for all licensed therapists.
• Bachelor degree in Physical Therapy Program (reflecting PT credential).
• Availability on weekends is a must.

Responsibilities

• Primary responsibility is to ensure residents receive a level of treatment consistent with Care Plans, monitoring RUGS levels and preparing billing.
• Collaborate with interdisciplinary team to achieve resident goals and ensure quality care.

Physical Therapist PT Shift: Day shift

Physical Therapist PT Full TIME

The appropriate candidates will join our active Sub-acute care program and Rehabilitation Services which are "In-House" and provided by our interdisciplinary team of staff therapists. Primary responsibility is to ensure residents receive a level of treatment consistent with Care Plans, monitoring RUGS levels and preparing billing.
