Job summary

PurposeCare is seeking a dedicated Physical Therapist to join our home health team in the Midwest. You will perform skilled visits in patients' homes, make independent clinical decisions, and collaborate with the care team to deliver holistic, patient-centered care.

Strong emphasis on autonomy, compassionate patient interactions, and maintaining safety in home environments. Qualifications include an active Physical Therapist license in Indiana and experience with home health and OASIS preferred.

Qualifications

• Current Physical Therapist license in Indiana is required.
• Experience with home health and OASIS is preferred.
• Ability to perform tasks involving physical activity (up to 50 lbs) and drive to patient homes.

Responsibilities

• Provide skilled home visits and address the full scope of patient needs.
• Make independent clinical decisions to ensure optimal patient outcomes.
• Establish meaningful, one-on-one connections with patients in their homes.

Qualifications and Requirements:

• Current Physical Therapist license in the state in which you are applying
• 1 year of physical therapist experience is preferred - home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check
