Job summary

PurposeCare, a leading home health provider in the Midwest, is seeking a Physical Therapist Assistant to join our home-based care team in Indiana. You will deliver skilled visits, assess patient needs, and support recovery in patients’ homes.

You’ll enjoy flexible scheduling, autonomy in clinical decisions, manageable caseloads, and opportunities for professional development while helping clients live healthier, safer lives at home.

Qualifications

• Current Physical Therapist Assistant license in the state you are applying
• 1 year of physical therapy assistant experience preferred - home health experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Responsibilities

• Provide skilled PT visits that address the full scope of a patient's needs in their home
• Build meaningful, one-on-one connections with patients in their homes
• Deliver compassionate, high-quality care focused on safe recovery and independence
