PurposeCare is seeking a Physical Therapist to join our family in La Porte, Indiana. We provide home health services and are dedicated to making a difference in the lives of our clients. As a Physical Therapist, you will enjoy flexible scheduling and the ability to provide personalized care to patients in their homes.

We offer a competitive pay structure with rates ranging from $72 to $115 per visit. Join us in our mission to bring healthcare home and help clients thrive in their communities.

Formación

• 1 year of physical therapist experience is preferred.
• Ability to perform tasks involving physical activity.
• Current Driver's License and vehicle insurance required.

Qualifications and Requirements:

• Current Physical Therapist license in the state in which you are applying
• 1 year of physical therapist experience is preferred - home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Hiring Range

This is a paid per visit position - Rates range from $72 - $115
