PurposeCare is seeking a Physical Therapist to join our home health team in Michigan. This role offers per-visit compensation and the chance to deliver skilled therapy in patients’ homes, promoting independence and recovery with individualized plans.

Applicants must hold a current PT license in the applying state, with 1 year of PT experience preferred, especially in home health and OASIS workflows. Mileage support and comprehensive benefits are provided.

Qualifications

• Current Physical Therapy license in the state where you apply.
• 1 year of physical therapist experience preferred; home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs.

Qualifications and Requirements:

• Current Physical Therapist license in the state in which you are applying
• 1 year of physical therapist experience is preferred - home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Hiring Range

This is a paid per visit position - Rates range from $72 - $77 base
