Job summary

Aspire Home Healthcare is seeking a dedicated Physical Therapist (PT) to provide in-home, one-on-one care across Ramona and surrounding areas. You will evaluate, plan, and implement individualized home health treatment plans to help patients regain independence.

We offer flexible scheduling, competitive per-visit rates starting at $85, mileage reimbursement, and opportunities for growth across multiple states.

Qualifications

• Active Physical Therapist (PT) license in CA.
• Minimum 1 year of PT experience preferred.
• Physical exam within the last 6 months.
• TB test within 12 months and CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patient condition and establish measurable treatment goals.
• Develop and implement individualized treatment plans.
• Supervise and coordinate care with PTAs and HHAs.
• Document patient progress, updates, and treatment notes.
• Educate patients and families on exercises, mobility, and use of adaptive equipment.
