Job summary

Aspire Home Healthcare is seeking a licensed Physical Therapist (CA) to provide one-on-one PT in patients' homes in North County Inland. The role supports flexible full-time or part-time schedules with competitive per-visit rates starting at $85.00, mileage reimbursement, and opportunities for growth across states.

You will evaluate and treat patients, coordinate care with rehab teams, educate families, and document progress while maintaining HIPAA standards and quality care.

Qualifications

• Active Physical Therapist license in CA with current credentials.
• One year of PT experience preferred in home health or clinical settings.
• CPR certification and TB test current within required timeframes.

Responsibilities

• Provide individualized home health PT care and document progress.
• Evaluate patient condition and establish measurable goals.
• Develop and implement treatment plans and adjust as needed.
• Supervise PTAs and coordinate care with HHAs and nurses.
• Educate patients and families on exercises and safety at home.
• Ensure HIPAA compliance and accurate documentation.
