Aspire Home Healthcare in San Diego, CA is seeking a skilled Physical Therapist (PT) for home health care. The role involves evaluating, planning, and delivering one-on-one treatment programs for home health patients in collaboration with the rehabilitation team.

Qualifications

• Active Physical Therapist (PT) license in CA.
• Minimum 1 year of PT experience preferred.
• Physical exam within the last 6 months.
• Current TB test (within 12 months) and CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide effective therapy using approved techniques and procedures.
• Evaluate patient condition and set measurable goals.
• Develop and implement individualized treatment plans.
• Supervise and coordinate care with PTAs and HHAs.
• Document progress accurately and on time; educate patients.
• Evaluate home environments and recommend safety improvements.
• Ensure HIPAA confidentiality in all services.
• Coordinate with nursing, social work, and clinical staff for comprehensive care.

Your Role:

The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team.

Key Responsibilities:

• Provide effective patient care using approved therapy techniques and procedures
• Evaluate patient condition and establish measurable treatment goals
• Develop and implement individualized treatment plans
• Supervise and coordinate care with PTAs and HHAs
• Document patient progress, updates, and treatment notes accurately and on timeEducate patients and families on exercises, mobility, and use of adaptive equipment
• Evaluate home environments and recommend modifications to improve safety and independence
• Ensure all services meet HIPAA standards for confidentiality
• Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
