Job summary

Aspire Home Healthcare of Northern California – Yuba City, CA 95991 is hiring a part-time or full-time Physical Therapist Assistant (PTA) to serve patients in home health settings. You will deliver therapy under the supervision of a licensed Physical Therapist, helping patients regain mobility and independence in their own homes.

The role emphasizes one-on-one patient care, adherence to the Plan of Care, accurate progress notes, and HIPAA compliance.

Qualifications

• Licensed Physical Therapist Assistant (PTA) in CA
• Minimum of 1 year of PT/PTA experience preferred
• Experience with OASIS and WellSky (Kinnser) strongly preferred
• Current Physical Exam (within last 6 months)
• Current TB test (within last 12 months) and CPR certification
• Valid driver’s license and reliable transportation
• Ability to attend a 1-day orientation at our Yuba City office

Responsibilities

• Follow and implement the Plan of Care as established by the supervising Physical Therapist
• Prepare accurate daily progress notes for treatments provided (to be reviewed and signed by the PT)
• Maintain patient confidentiality per HIPAA regulations
• Submit all visit records and documentation in a timely manner
• Attend in-service and on-the-job training sessions as required
• Follow Aspire Home Healthcare’s established policies and procedures
