Total Care Therapy is seeking a compassionate Physical Therapist to join our growing Toledo team. You will evaluate residents, create individualized plans of care, and deliver treatments to improve mobility, reduce pain, and restore function.

Qualifications

• Current State Physical Therapist License required.
• CPR/BLS Certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including exercise, gait and balance training.
• Reassess progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations.
• Complete timely documentation in EMR.
• Educate residents and staff on home exercise programs and fall prevention.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload with quality outcomes.
