Job summary

Total Care Therapy in Ohio is seeking a compassionate Physical Therapist to evaluate, develop, and implement personalized treatment plans that improve mobility and independence for residents in assisted living and independent living communities.

You will lead skilled PT interventions, monitor progress, supervise PTAs where allowed, document in EMR, and educate residents and staff while collaborating with an interdisciplinary team to deliver high-quality, patient-centered care.

Qualifications

• Current State Physical Therapist (PT) License is required.
• CPR/BLS Certification is required.
• Valid driver's license with reliable transportation and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations.
• Complete timely and accurate EMR documentation.
• Educate residents, caregivers, and staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team for coordinated patient-centered care.
• Maintain a productive clinical caseload delivering exceptional therapy services.
