Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in assisted living and independent living communities in Schererville, IN. You will evaluate residents, develop individualized treatment plans, and help residents improve mobility, reduce pain, and maintain independence.

Qualifications

• Current State Physical Therapist license.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait, balance, transfer, and mobility training.
• Reassess progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Complete timely and accurate documentation in the EMR.
• Educate residents, caregivers, and staff on treatment plans and home exercises.
• Collaborate with interdisciplinary team for coordinated care.
