Job summary

Evolve Therapy Services in Paterson, NJ seeks a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility. You will assist the Physical Therapist in delivering high-quality resident care and implementing treatment plans in a supportive team setting.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and in developing treatment plans with the Physical Therapist.
• Assist in establishing short- and long-term goals and treatment plans.
• Deliver individual and group treatment across mobility, strength, and gait activities.
• Document CMS-compliant notes and progress in NetHealth EMR.
