Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Trenton, NJ. This full-time position focuses on delivering quality resident care under the supervision of a Physical Therapist, with emphasis on evaluating needs and implementing treatment plans.

Candidates should hold an Associate's degree in physical therapy and an active PTA license in the state, with commitment to ongoing education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine intervention needs.
• Assist in evaluating residents and developing treatment plans with the PT.
• Implement treatment plans including mobility, strength, balance and gait training.
• Train residents, families and staff in use of therapy equipment.
• Maintain compliant documentation in NetHealth EMR as required.
