Responsibilities

• Provide education and counseling to patient/family about treatments and goals.
• Evaluate physical and functional status to determine patient capabilities.
• Document evaluations, treatment, and coordination of care per facility policies.
• Independently manage own caseload to maximize effectiveness of care plan.
• Comply with applicable laws, regulations, and facility policies.
