AIDS Healthcare Foundation is seeking a Patient Care Manager to lead psychosocial case management at the Westside HCC Healthcare Center in Los Angeles. The role focuses on assessing social determinants of health, coordinating services, and guiding patients through Ryan White program eligibility and community resources.

Responsibilities include developing Care Plans, collaborating with providers, and engaging patients to reach health and social goals, with a hybrid work option after 90 days.

Qualifications

• Master's degree in Social Work, Counseling, Psychology, Marriage/Family Counseling or Human Services.
• Experience with psychosocial case management and social determinants of health.
• Familiarity with the Ryan White Care Act and HAB measures.
• Ability to develop care/service plans and coordinate referrals.

Responsabilités

• Perform psychosocial case management and health risk/needs assessments.
• Develop Service/Action/Care Plans related to psychosocial needs.
• Coordinate referrals and support to community resources.
