Qualifications

• Must perform initial assessments and document findings.
• Collect and analyze admission data.
• Involve patient and family in assessment and documentation.
• Advocate for patients and protect privacy and rights.
• Develop and update plan of care with multidisciplinary team.
• Monitor and document patient status; adjust plans as needed.
• Identify safety risks and act to prevent harm.
• Provide patient education and discharge planning.

Responsibilities

• Complete initial assessments and documents as required.
• Collect, record, and analyze pertinent admission data within prescribed timeframe.
• Involve patient’s support systems in assessment and documentation.
• Act as an advocate for patients; explain rights and obtain signatures.
• Initiate and update treatment plan and documentation.
• Identify health data and consult with team; update treatment plan accordingly.
• Implement patient care; ensure accurate medication administration and documentation.
• Identify potential care problems and intervene to prevent adverse outcomes.
• Provide health education for patient and family before discharge.
• Carry out leadership roles in unit operations and patient care.
