Responsibilities

• Complete initial assessments and document as required.
• Collect, record, and analyze data for admission assessments.
• Involve patient's support systems in assessment and documentation.
• Act as an advocate for patients and obtain necessary consents.
• Initiate and update treatment plans and documentation.
• Identify safety risks and intervene to prevent adverse outcomes.
• Administer medications safely and educate patients about medications.
• Provide discharge planning and health education.
