Job summary

Warren Park Health & Living Center is seeking a Psychiatric Rehabilitation Service Assistant to join their team. The role provides intervention services to residents, including personal care, psychosocial training, and social integration.

The position is unionized and requires reading and writing in English, with experience with mentally ill residents, and completion of approved training programs in psychiatric rehabilitation.

Qualifications

• High school diploma or GED preferred.
• At least one year of experience with mentally ill residents preferred.
• Completion of a training program approved per 77 Ill. Adm. Code 395 is required.
• Completion of a psychiatric rehabilitation certificate program with expertise in the principles.

Responsibilities

• Participates in new resident orientation and helps acclimate residents.
• Contributes to MDS completion and overall assessment.
• Conducts individual and group psychosocial training and counseling, documenting progress.
• Contributes to the resident's individual treatment plan.
• Inspects residents' rooms and belongings for hygiene and safety; documents issues.
• Assists residents with community outings and punctual arrivals.
• Offers residents a space to talk and seek support with staff.
• Supports training program implementation and evaluates outcomes.
• Maintains knowledge of goals and programs relevant to residents.
• Leads group counseling sessions and encourages participation.
• All other duties as assigned.
