Job summary

Warren Park Health & Living Center in Chicago is seeking a Psychiatric Rehabilitation Service Assistant to provide intervention services to residents, including personal care and psychosocial training.

This is a collectively bargained position under the union. You will participate in orientation, contribute to assessments and treatment plans, lead group counseling, document progress, assist with outings, and support residents’ social integration.

Qualifications

• Ability to read, write, and communicate in English
• Relate positively with patients, families, and staff
• Strong verbal and written communication to perform tasks
• Ability to lead individual and group trainings successfully

Responsibilities

• Participates in new resident orientation and acclimation
• Contributes to MDS and overall assessments
• Conducts individual and group psychosocial training and counseling with progress documentation
• Contributes to the resident’s individual treatment plan
• Inspects and reviews residents and rooms for hygiene and safety
• Documents problems/deficiencies after inspections
• Assists residents on community outings and arrivals on time
• Provides support for self-expression and resident talk sessions
• Assists in implementing in-service training programs and policy evaluation
• Keeps knowledge of facility goals and programs up to date
• Leads group counseling sessions and encourages participation
• Performs all other duties as assigned

Job description

Warren Park Health & Living Center is seeking a Psychiatric Rehabilitation Service Assistant to join their team! The Psychiatric Rehabilitation Service Assistant is responsible for providing necessary intervention services to residents. These services include: personal care, individual and group psychosocial training and supportive social integration. At this facility, this is a collectively bargained position under the union.

Psychiatric Rehabilitation Service Assistant Duties and Responsibilities:

• Participates in new resident orientation, helping the resident become acclimated to the facility.
• Contributes to the completion of the MDS and overall assessment.
• Conducts individual and group psychosocial training and counseling sessions, including documenting progress.
• Contributes to the resident’s individual treatment plan.
• Participates in inspection and review of residents and their rooms, clothing and possessions to insure that each is well-groomed, in good hygiene and rooms, clothing and possessions are adequately and safely maintained.
• Documents problems/deficiencies following inspection.
• Assists residents going on community outings with dressing appropriately and arriving at the designated departure area at the scheduled time.
• Is available for residents who identify a need to talk and meet with a staff person for the purpose of self-expression and support.
• Works with manager to assist in the implementation of in-service training programs; contributes ideas for program policies and procedures; and assists in the evaluation of program outcomes and recommends program changes.
• Maintains current and accurate knowledge of the goals and programs relevant to residents in the facility.
• Leads group counseling sessions, as assigned, encouraging residents to participate.
• All other duties as assigned.

Psychiatric Rehabilitation Service Assistant DutiesQualifications and Skills:

• Ability to read, write, and communicate in English.
• Ability to relate positively, effectively, and appropriately and independently with patients, families, and other staff members.
• Strong verbal and written communication skills to perform the tasks required.
• Ability to lead both individual and group trainings successfully as measured by residents’ improvement.

Psychiatric Rehabilitation Service Assistant Duties Education and Experience:

• High school diploma or GED, preferred.
• At least one year of experience with mentally ill residents, preferred.
• Successful completion of a training program approved in accordance with the Long-Term Care Assistants and Aides Training Programs Code (77 Ill. Adm. Code 395), required.
• Completion of a psychiatric rehabilitation certificate program taught by a professional entity/company with expertise in psychiatric rehabilitation principles.

Physical Requirements:

• Must be able to move intermittently throughout the workday.
• May be necessary to assist in the evacuation of residents during emergency situationsLimited potential for exposure to environmental hazards.
