Job summary

Warren Park Health & Living Center is seeking a Psychiatric Rehabilitation Service Assistant to provide intervention services including personal care and psychosocial training for residents. This is a collectively bargained position under the union.

The role involves leading group and individual trainings, assisting in treatment planning, and coordinating with staff to ensure resident well-being and progress. Strong communication and advocacy skills are essential.

Qualifications

• High school diploma or GED preferred.
• At least one year of experience with mentally ill residents preferred.
• Successful completion of a training program approved per 77 Ill. Adm. Code 395 required.
• Completion of a psychiatric rehabilitation certificate program taught by a professional entity with expertise in psychiatric rehabilitation principles.

Responsibilities

• Participates in new resident orientation to help acclimate residents.
• Contributes to MDS and overall assessment completion.
• Conducts individual and group psychosocial training and counseling, documenting progress.
• Contributes to the resident’s individual treatment plan.
• Inspects residents and rooms to ensure hygiene and safety; documents deficiencies.
• Assists residents on community outings and ensures timely arrival.
• Provides support for residents to talk and express themselves to staff.
• Works with management to implement in-service training and evaluate programs.
• Maintains current knowledge of goals and programs relevant to residents.
• Leads group counseling sessions as assigned to encourage participation.
