Job summary

WES Health System seeks a Psychiatric Rehabilitation Worker to provide rehabilitation services in our facility or community settings. You will facilitate groups, support individual skill development, and help participants access services to live in their chosen environment.

You will assist with rehabilitation plans, document progress daily, and collaborate with the team to ensure safe, person-centered care.

Qualifications

• Associates degree with at least one year in mental health direct services.
• CPS certification or two years of volunteer or paid experience in human services (one year in mental health direct services) OR high school diploma with two years in human services.

Responsibilities

• Provide psychiatric rehabilitation services in facility or community.
• Facilitate groups and individual supports; document daily progress.
• Support development of individualized rehabilitation plans.
• Promote community integration and leisure skills.
• Maintain caseload and participate in crisis response.

JOB TITLE: Psychiatric Rehabilitation Worker

Creative Therapeutic Specialties such as Music, Art, Dance and Recreation a plus!

Salary Range: $35,000-$40,000 dependent on experience.

GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES

• To provide Psychiatric Rehabilitation Services facility based and/or in the community as assigned
• To provide groups/unit to enhance an individual's knowledge to develop, enhance and retain skills and competencies in living, learning, working, and socializing
• To help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice
• To support PRS staff with service provision; participate in the development of individualized rehabilitation plans, group facilitation and daily documentation
• Provide goal directed community integrative activities to support the development of participant leisure and social skills
• To provide support and direction to overall program planning and development
• To ensure the safety and wellbeing of program participants and staff
• To support the maintenance of a safe and open environment

ESSENTIAL & CORE FUNCTIONS

• Provides and facilitates individual's skill development and support which includes daily living, socialization, educational/vocational supports and opportunities, and leisure/recreation activities planning to include community integration and community connections
• Primary responsibility for facilitation of groups, individual supports to members and documentation
• Assists with special projects, reports and program planning
• Provide support in program planning and development sessions as well as practice guidelines and standards
• Maintains and monitors an assigned caseload of individual
• Participates in the analysis and identification of service delivery gaps, changes and problems under the guidance of the supervisor
• Participates in the resolution of crisis situations; assist with identifying individuals and resources that can provide direct support and/or immediate intervention during a crisis
• Establishes and maintains liaisons with other service providers to promote program coordination and enhance individual services
• Participates in relevant training to enhance professional growth, and development with an emphasis on psychiatric rehabilitation services and evidence-based practices
• Provide supplemental coverage and/or fill-in for other programmatic services as needed
• Serves as an advocate and encourages individuals to assume a proactive role in their rehabilitative process, including a mastery of skills
• Attends staff and other meetings as they relate to the individuals and their overall growth and development
• Performs other duties and responsibilities as they relate to the overall enhancement of program service delivery
• May require crisis intervention for emergency situations

ADDITIONAL RESPONSIBILITIES

• Performs other duties and special projects assigned

PREREQUISITES & QUALIFICATIONS FOR THE POSITION

• Associates degree and One (1) year of work experience in the mental health direct service,
• CPS certification one (1) year of volunteering or paid,
• or high School diploma/GED and two years' work experience in Human Services, which one year must be in mental health direct services

OR

• CPRP certification

COMPETENCIES & PERSONAL CHARACTERISTICS

• Must have competent oral and written communication skills, as well as be detail-oriented and able to work in a fast-paced environment
• Strong computer skills are required, as well as internet research abilities
• Ability to work independently and as part of a team
• Knowledge of mental health system and supports
• Must be available for local travel.
• Participation in on call coverage as applicable
• May be required to report to work during emergencies including inclement weather

ORGANIZATIONAL ACCOUNTABILITY & RELATIONSHIPS

• The individual is to be supervised by the Program Manager /Lead Psychiatric Rehabilitation Specialist
